Google Forms with folders list [closed]

I currently have a folder on Google Drive with 20 students each having their own documents in their own folders.

I am wanting to create a google form, the form would have a drop down list of the student names (from the folder names), to record details of say incidents within the class that then uploads to that child's folder - with date and time stamp or to a spreadsheet detailing the same.
